Why This Matters
Interstitial lung disease (ILD) is an underrecognized extraintestinal complication in IBD that in this study was relatively common (12%) and associated with a substantially increased risk of death.
Patients and clinicians should be aware of possible lung involvement, especially when other risk factors (smoking, higher BMI, bronchiectasis, older age, exposure to certain biologics) are present.
Who Should Pay Attention
Adult IBD patients (including Crohn's and UC), patients on anti-TNF biologics, gastroenterologists, pulmonologists, and researchers of IBD extraintestinal manifestations.

What To Know
This study reviewed electronic health records and radiology-confirmed ILD between 2013–2023 and used both cohort and matched case–control analyses. ILD patterns varied (nonspecific interstitial pneumonia, usual interstitial pneumonia, indeterminate), and pulmonary function testing often showed restriction and reduced diffusing capacity.
The authors suggest early identification, routine respiratory assessment, and multidisciplinary care for IBD patients.

Findings come from a retrospective EHR-based cohort at a single center with radiology-confirmed ILD; observational design limits causal inference and anti-TNF association may reflect confounding by disease severity. The report supports consideration of respiratory assessment but does not establish screening guidelines.
