Why This Matters

The findings give clinicians and patients data on how often biologic therapy leads to endoscopic remission in biologic‑naïve patients with isolated small‑bowel Crohn's disease, and they suggest similar effectiveness between infliximab and ustekinumab after matching.

Who Should Pay Attention

Clinicians treating small‑bowel Crohn's disease, researchers studying biologic effectiveness in isolated SB‑CD, and adult patients considering biologic therapy (especially biologic‑naïve patients).

What To Know

The study included 169 biologic‑naïve patients with isolated SB‑CD who started IFX (n=122) or UST (n=47) and had follow-up double‑balloon enteroscopy. ER was defined as SES‑CD <3. Overall ER was 35.5% (32.8% IFX, 42.6% UST).

Multivariate analysis found that stricturing/penetrating disease and moderate-to-severe endoscopic activity were associated with lower odds of ER. After propensity matching, IFX and UST showed no significant differences in endoscopic response, ER, mucosal healing, clinical remission, or biochemical remission.

Keep In Mind

Results come from a retrospective single‑center cohort with double‑balloon enteroscopy follow-up; abstract-level data do not substitute for randomized trials. The study timeframe was 2020–2025 and used SES‑CD <3 to define endoscopic remission.
