Why This Matters

Patients and clinicians choosing a next biologic after prior biologic failure may find evidence here that effectiveness is broadly similar between vedolizumab and ustekinumab overall, with potential differences depending on how many prior biologics a patient received.

Who Should Pay Attention

Adult patients with ulcerative colitis who have previously used biologic therapy; clinicians deciding on subsequent biologic sequencing; researchers studying real-world biologic effectiveness.

What To Know

This real-world analysis pooled data from 420 biologic-experienced UC patients treated between 2020–2024 across 14 centers. Primary outcomes were clinical remission and steroid-free clinical remission at weeks 14–16, 52, and 104, using statistical weighting to adjust baseline differences.

At group level the two drugs had similar rates of steroid-free clinical remission at most timepoints. At week 104 vedolizumab showed a higher clinical remission rate but without a corresponding difference in steroid-free remission, and the authors advise cautious interpretation because of limits in long-term follow-up.

In exploratory subgroup analyses, vedolizumab appeared more favorable in patients with a single prior biologic, while ustekinumab showed better outcomes in patients with two or more prior biologics; ustekinumab also had higher treatment persistence. Safety profiles were reported as comparable between treatments.

Keep In Mind

This is a retrospective, observational multicenter study (not a randomized trial). Subgroup analyses were exploratory and long-term (104-week) findings have limitations related to follow-up and potential residual confounding despite weighting adjustments.
