Why This Matters

This research examines whether LLMs that cite real PubMed papers actually give clinically reliable answers for perioperative IBD questions. Patients and clinicians should know that authentic citations don’t automatically mean safe or accurate medical recommendations.

Who Should Pay Attention

Clinicians using or evaluating LLM tools for IBD perioperative care, informaticians and researchers developing medical RAG systems, and health systems considering on-premises LLM deployment.

What To Know

This study tested whether adding full-text PubMed retrieval (RAG) to locally run 8B LLMs improves clinically reliable answers on a difficult 50-question perioperative IBD benchmark. Reviewers confirmed that all retrieved PubMed IDs matched real PubMed records, so the system reliably cited authentic sources.

RAG improved accuracy for one model (Qwen3) but not the other, did not reliably improve patient-safety ratings, and in one model reduced faithfulness (consistency with sources). The authors conclude that having authentic references does not guarantee clinically correct or safe outputs and suggest external safety checks are needed for high-risk use.

Keep it practical: this is a controlled evaluation of model behavior with an IBD perioperative question set, not a clinical trial of a medical intervention. It speaks to limits of LLM retrieval systems and to the need for independent safety validation before clinical deployment.

Keep In Mind

Structured-content depth is “abstract” from the journal: the summary and results are drawn from the article abstract rather than a full independent review. The paper evaluates model outputs on a benchmark and expert ratings; it does not test patient outcomes or real-world clinical deployment.
