Why This Matters

Choosing which biologic to start for ulcerative colitis appears to affect how much time patients spend in corticosteroid-free remission over the first 24 months. That could influence quality of life and future treatment planning.

Who Should Pay Attention

Adult patients with UC considering a first biologic, clinicians who prescribe biologics, and researchers studying biologic sequencing and real-world effectiveness.

What To Know

This study compared two real-world first-line biologic selection strategies for adults with active ulcerative colitis: an effectiveness-based strategy (EBS) using infliximab or vedolizumab, and an acceptability-based strategy (ABS) using adalimumab or golimumab.

Over follow-up, patients starting with EBS spent a larger percentage of months in corticosteroid-free patient-reported clinical remission (PRO2-CFREM) within the first 24 months than those starting with ABS. The authors analyzed 130 patients and adjusted comparisons with propensity scores.

The main take-home reported by the abstract is that initial choice of biologic materially affected outcomes and later switching did not fully compensate for a less-effective first choice. Who this matters to: if you or a patient is starting a first biologic for UC, this paper suggests the specific first agent may influence long-term remission time.

Discussing relative effectiveness and treatment goals with a clinician when choosing a first biologic is reasonable.

Keep In Mind

This is an observational real-world study summarized in the journal abstract. Propensity-score adjustment was used, but observational studies can have residual confounding. The abstract does not replace a full-text review for treatment decisions.
