What To Know

This study reports 52-week real-world outcomes for mirikizumab, an anti–IL-23p19 biologic, in 120 adults with active ulcerative colitis treated at two international centers. Patients were largely treatment-experienced, with high prior exposure to anti-TNF agents, vedolizumab, and JAK inhibitors.

The study measured clinical response, remission, corticosteroid-free remission, drug persistence, and safety through 52 weeks. At one year, clinical remission (SCCAI ≤2) was reported in 28.3% of patients (35.6% by a modified non-responder imputation), with drug persistence of 64.6%. Corticosteroid-free remission at 52 weeks was 26.1%.

Adverse events were reported in 10.8% of patients and led to discontinuation in 4.2%. The cohort included a subgroup who received extended induction; their week-26 remission rate was lower than those receiving standard induction. These results come from a retrospective observational cohort (real-world) and extend prior 12-week data from the same centers.

The report supports mirikizumab’s effectiveness and tolerability in a treatment-experienced UC population but does not replace randomized trial evidence or individualized medical advice.
