If epithelial cells themselves retain disease-linked programs, that could help explain persistent intestinal abnormalities in Crohn’s disease even during remission and point to new biomarker or therapeutic targets focused on the epithelium or on metabolite–epithelial interactions.

The study used donor-matched intestinal biopsies and derived epithelial-enriched organoids from patients with active and inactive Crohn’s disease and from controls, then applied single-cell RNA-seq and ATAC-seq to tissue and organoid cells.
Organoids from Crohn’s patients retained autonomous disease-associated epithelial states, while tissue signatures additionally reflected immune and microbial interactions. When exposed to luminal metabolites from patients, Crohn’s organoids showed selective activation of disease-related transcriptional programs and chromatin remodeling.
The work is preclinical and mechanistic: it aims to separate cell-intrinsic epithelial alterations from environmental influences (microbial/metabolite cues) using organoid models and single-cell genomics. The findings suggest both intrinsic epithelial dysregulation and microenvironmental signals contribute to persistent disease-associated states.
