What To Know

This small case series reports 7 people with IBD (2 Crohn's, 5 UC) who developed hepatitis E infection while on biologic or small-molecule therapy.

In all cases the infection was detected after new elevations in liver enzymes, was asymptomatic or mild, and resolved without liver dysfunction; hepatitis E RNA became undetectable by week 12 and transaminases normalized (median ~4 weeks). Advanced IBD therapy was paused and later resumed without reported complications.

One pregnant patient had an uncomplicated course and a healthy delivery. The report is an abstract-level case series (no randomized comparison). It suggests that hepatitis E can present as transient transaminase elevations in immunosuppressed IBD patients and may not always progress to chronic infection, but numbers are small and follow-up is limited.

If you have IBD and rising liver enzymes while on advanced therapy, your care team may test for hepatitis E virus (serology and PCR) as part of the workup and may consider temporarily holding immunosuppression until viremia clears. This study does not replace individualized medical advice.
