This Scientific Reports analysis used Brazil’s GEDIIB national IBD registry (June 2023–June 2024) to identify clinical features linked with “advanced therapy optimization” (use and dose/strategy changes of biologic or small-molecule therapies) in Crohn’s disease (CD) and ulcerative colitis (UC).

The authors report higher use and more frequent optimization of advanced therapies in CD than UC and describe patient-level factors associated with optimization in each disease.

In Crohn’s disease, younger age at onset (A1 and A2), presence of perianal disease, and prior abdominal surgery involving an ileal pouch were independently associated with greater prevalence of optimization.

In ulcerative colitis, prior surgery, chronic liver disease, and prior herpes zoster infection were associated with higher prevalence of optimization on multivariable analysis. These findings are registry-based and observational: they describe associations rather than proving causation.

The study may help clinicians and patients understand which subgroups more often require intensified advanced-treatment strategies and could inform discussions about monitoring and treatment planning.
