Why This Matters

Hospitalized patients with moderate-to-severe ulcerative colitis flares sometimes need escalation to biologic or surgical treatments. This trial is testing whether a short course of HBOT can improve symptoms and reduce the need for in-hospital escalation by day 5, and whether immune–microbe changes explain benefit.

Who Should Pay Attention

Adults hospitalized for moderate-to-severe ulcerative colitis flares, IBD clinicians managing inpatients, and researchers interested in HBOT, mucosal hypoxia, or immune–microbiome mechanisms.

What To Know

This ClinicalTrials.gov record describes a randomized, sham-controlled interventional trial testing hyperbaric oxygen therapy (HBOT) for hospitalized adults with moderate-to-severe ulcerative colitis flares. The active treatment is 2.4 ATA 100% oxygen for 90 minutes once daily for 5 days; the control is a sham compression with room air.

Primary outcome is clinical response by day 5 (resolution of rectal bleeding and improved stool frequency) without need for in-hospital biologics, small molecules, or colectomy. The trial also plans to study immune–microbe mechanisms of response. The registry status is recruiting with planned enrollment of 126 across multiple US centers.

Keep In Mind

This is a registered interventional trial (recruiting). Previous small randomized trials reported benefit, and this larger study aims to confirm those findings and examine mechanisms. As a trial record, it describes planned interventions and outcomes but does not report results.
