What To Know

This multicenter prospective study (VedoKids) tested whether serum metabolite profiles predict clinical remission after vedolizumab in children with IBD.

Researchers measured metabolites at baseline, week 14, and week 30 and used machine-learning models (random forest with feature selection) combining clinical and metabolomic data to predict remission at the next visit. The study included 138 children (mean age ~13.6 years) with Crohn’s disease or ulcerative colitis.

Models showed good-to-strong performance (AUROC ranged ~0.72–0.92 depending on disease and timepoint). Specific metabolites (for CD: PC ae C36:4, citric acid, malic acid; for UC: LysoPC a C26:1, picolinic acid, serotonin) were among top predictive features.

This is an abstract-level report of a prospective cohort study; it describes promising biomarker associations but does not establish clinical utility or that metabolite testing is ready for routine care.
