What To Know

This pathology study compared morphologic patterns of colorectal dysplasia in patients without inflammatory bowel disease (sporadic lesions) to dysplasia from patients with IBD using hundreds of archived lesions.

The authors found that most sporadic dysplastic lesions were conventional, low-grade, polypoid, and right-sided, while nonconventional patterns were less common than in IBD-associated lesions. Several nonconventional patterns (for example, hypermucinous and goblet cell–deficient dysplasia) were rare or absent in the non-IBD group.

The paper notes that nonconventional dysplasia is not exclusive to IBD but appears less prevalent and has a narrower morphologic spectrum in patients without IBD.

The authors caution that because routine random colonic biopsies are uncommon in the non-IBD population, the study cannot fully exclude an association between nonconventional patterns and concurrent or future colorectal cancer.
