This single-center retrospective series enrolled 32 pediatric-onset Crohn's patients treated with adalimumab and compared those in clinical remission with those who experienced loss of response.

Reactive TDM (measuring trough levels and ADAs) was used after clinical loss of response to categorize mechanisms: some patients had therapeutic drug levels without ADAs and could remain on adalimumab; others had subtherapeutic levels or detectable ADAs prompting dose escalation or switching to another biologic.
