Why This Matters

Researchers and clinicians want biomarkers that predict outcome or tailor treatment. This study suggests a mucosal gene-expression score is prognostic and declines in responders, but it does not yet show the score changes who benefits from biologic therapy.

Who Should Pay Attention

Clinicians, researchers, and patients on biologics interested in biomarker-driven prognostication and trial-based molecular analyses.

What To Know

The paper analyzed public trial datasets to separate prognostic association (score predicts outcome regardless of treatment) from treatment-effect modification (score changes who benefits). Across seven treated cohorts higher baseline score was linked with worse outcomes, and paired samples showed responders had larger declines in the score over time.

Randomized interaction analyses in three active-versus-placebo trials produced interaction estimates below 1 but with wide confidence intervals crossing 1, so they do not establish that the score changes relative treatment benefit.

The authors note limited donor numbers in some compartment analyses and call for prospective independent validation before clinical use.

Keep In Mind

Findings are based on re-analysis of public trial datasets and an abstract-level summary; randomized interaction tests were inconclusive and the authors state prospective independent validation is needed before clinical use.
