The researchers looked specifically at IBD patients to reduce heterogeneity seen in prior IMID studies. After one vaccine dose, IBD patients showed fewer Th1-polarized spike-reactive T cells and more Th2 cells versus healthy controls. There was also increased waning of spike-specific cytokine responses after two doses.
Patients treated with anti-TNF or anti-IL-12/IL-23 biologics had lower spike-specific IL-2 secretion than untreated IBD patients following one dose.
However, single-cell RNA sequencing of spike-responsive T cells collected 2–4 weeks after two or three doses found no major differences in T cell subset composition, transcriptomes, or TCR diversity between treated patients and healthy controls.
Bottom line: the paper suggests some impairment in early or waning T cell responses in IBD patients — and some treatment-related effects after a single dose — but after a full vaccine course spike-reactive T cells in biologic-treated patients appeared similar to healthy controls.
