Why This Matters

Simple, routinely measured nutritional scores were associated with early (14-week) remission on vedolizumab in patients with moderate-to-severe UC, which could help identify patients more likely to respond early to this biologic.

Who Should Pay Attention

Adult patients with moderate-to-severe ulcerative colitis, gastroenterologists and IBD clinicians considering vedolizumab, and researchers studying predictors of biologic response.

What To Know

This retrospective multicenter study from northern China included 184 patients with moderate-to-severe UC treated with vedolizumab and examined baseline nutritional scores alongside routine blood biomarkers.

GNRI showed the strongest correlation and the highest discriminatory ability (AUC 0.697) for predicting 14-week clinical remission; PNI and MUST were also associated. By later follow-up (weeks 30 and 54), only baseline hemoglobin remained independently associated with remission.

The findings suggest that lower nutritional risk (higher GNRI/PNI, lower MUST) may be linked to a greater chance of early clinical response to vedolizumab, but longer-term predictors differed. These results are hypothesis-generating and reflect associations from an observational, retrospective cohort rather than randomized evidence.

Keep In Mind

Retrospective design and single-region multicenter sample limit causal inference and broad generalizability; results should be validated prospectively in diverse populations.
