Why This Matters

An implanted vagus nerve stimulator maintained remission for some people with mild-to-moderate Crohn’s disease over many years and appeared safe in this small series, suggesting neuromodulation may be a potential nonpharmacologic approach worth further study.

Who Should Pay Attention

Adult Crohn’s patients (low-to-moderate activity), gastroenterologists/IBD clinicians, device researchers, and patients interested in non-drug therapies

What To Know

This is a decade-long observational follow-up of nine patients first implanted between 2012–2016; the report is presented by the authors as a proof of concept rather than proof of effectiveness because there was no control group, randomization, or statistical power.

Clinical courses differed widely: some patients did well on stimulation alone for years (three stayed in remission for ten years), others benefited temporarily before needing biologics or surgery, and patients with very active severe disease did poorly and required early surgery or biologic therapy.

Safety signals in this small group were encouraging—no device removals or local infections were reported, and adverse effects were mainly stimulation-related discomfort (including voice changes). Objective vagal-function markers and fecal calprotectin data were incomplete, limiting interpretation.

Keep In Mind

Small sample size (n=9), no control/sham group, missing biomarker data (fecal calprotectin), and heterogenous patient courses limit conclusions; this is proof-of-concept only.
