Why This Matters

Perianal fistulas are common and burdensome in Crohn’s disease; this large multicenter Korean study quantifies high rates of complex fistulas, frequent recurrence, and suggests biologic therapy is associated with fewer recurrences, which is directly relevant to patients and clinicians managing perianal disease.

Who Should Pay Attention

Adults with Crohn's disease and perianal fistulas; gastroenterologists and colorectal surgeons; researchers focused on perianal Crohn’s disease; patients and caregivers tracking treatment outcomes.

What To Know

This chart-review study included 487 patients treated for newly diagnosed perianal fistula after 2011 with at least one year follow-up. Most patients had complex disease and a majority underwent fistula-related procedures. One-third experienced recurrence with a median time to recurrence under one year.

The authors report that complications such as abscess and stricture and having had fistula-related surgery during recurrence were associated with higher recurrence risk, while use of biologic therapy was associated with lower recurrence risk (hazard ratios reported in the abstract).

The study reflects real-world treatment patterns and outcomes across 15 Korean centers and helps quantify the clinical burden of perianal fistulizing CD, but it does not establish causation between treatments and outcomes.

Keep In Mind

Retrospective multicenter chart reviews can identify associations but cannot prove cause-effect. The abstract reports hazard ratios but lacks randomized allocation and full treatment details; use this as descriptive, hypothesis-generating evidence rather than definitive proof.
