What To Know

This small, retrospective exploratory study looked at 20 children (age 6–18) newly diagnosed with ulcerative colitis who had magnetic resonance enterography (MRE) as part of their diagnostic work-up.

Researchers measured total psoas muscle area (tPMA) on axial T2 images at L3 to estimate skeletal muscle mass and defined ‘at risk of sarcopenia’ (<10th percentile) and ‘severe sarcopenia’ (≤3rd percentile). They found 65% at risk and 30% with severe sarcopenia at diagnosis.

Severe sarcopenia was linked with lower BMI, higher clinical activity (PUCAI), higher inflammatory markers (WBC, neutrophils, CRP), and lower albumin. The authors used MRE scans opportunistically to assess muscle without additional imaging. These results are preliminary and hypothesis-generating.

The cohort was small and retrospective, so findings need confirmation in larger prospective multicenter studies before changing clinical practice or using MRE-derived muscle metrics for risk stratification.

For now, the study highlights that low muscle mass and malnutrition can be present at UC diagnosis and that routine diagnostic MRE offers a way to measure muscle mass without extra tests.
