Why This Matters

The study explores whether people with UC in deep histologic remission can reduce or stop maintenance medications without losing remission — a question many patients and clinicians face because of medication costs, side effects, and long-term management trade-offs.

Who Should Pay Attention

Adults with ulcerative colitis in deep remission; IBD clinicians considering de-escalation; researchers studying remission and biomarkers.

What To Know

This is a prospective, partially randomized, patient-preference interventional trial recruiting at the University of Chicago. Eligible adults in deep remission (endoscopic and histologic quiescence) may choose to de-escalate therapy or be randomized if they have no strong preference.

De-escalation is defined as stepping down from biologic or small-molecule therapy to oral aminosalicylate therapy or complete discontinuation; patients on immunomodulators or aminosalicylates would stop therapy.

Participants will be followed with clinical assessments, fecal calprotectin and CRP, intestinal ultrasound, and endoscopy/biopsies; primary outcomes focus on sustained biochemical, sonographic, clinical, and endoscopic remission at 12 months.

The design prioritizes real-world applicability by allowing patient preference while preserving randomization for undecided participants.

Keep In Mind

This is a recruiting interventional trial at a single tertiary center with a partially randomized, patient-preference design. The record describes planned methods and outcomes but does not report results.
